CE Records Collection

We collect a Completed Exam (CE) record for every exam performed within a department.  CE records include the exam:  accession number, date, time, location (specific room), exam type, patient birth month/year, and the performing technologist.  These records are accessed and managed in QMS from the Examinations page. 

These records provide a statistical context for exam quality detraction feedback.  Whenever a quality deficiency becomes apparent, managers should determine root cause of the issue so that it may be effectively addressed by asking questions such as:

  • How frequently does this happen?
  • Does the rate of occurrence indicate a need for immediate action? 
  • Are there certain types of exams where this is recurrent? 
  • Is this a performance issue for this individual technologist or is this a recurrent problem for multiple technologists? 
  • Is remedial training the solution or do we have an error trap in our process design? 
  • Does this problem occur more frequently at one location than it does at other similar locations? 
  • If so, is this issue arising from a deficiency in the equipment?  Or might there be a high number of difficult pediatrics or geriatrics patients in the referral base? 
  • Is there a need for immobilization equipment or a need for training on the immobilization equipment that is available? 
  • Does this problem recur more frequently during a specific shift or time of day? 
  • Is this a problem that recurs on weekends?

We utilize Completed Exams records as the basis for analysis since they allow exams to be grouped and compared by like-characteristics.  By formulating rates and facilitating comparisons, QMS enables managers to answer questions presented above.  By formulating rates, QMS empowers managers to set goals for improvement and to track progress.
